Mini-extracorporeal Circulation and Off-pump Techniques Associated with Less Inflammatory Gene Expression as Compared to On-Pump in the 24-hour Postoperative Window Following Coronary Artery Bypass Grafting

چکیده
Method Patients (n = 102) undergoing isolated coronary bypass grafting were prospectively enrolled and divided into cohorts of OPCAB (n = 34), MECC (n = 34), and ONCAB (n = 34). Serial blood samples were collected at multiple time points (preoperatively, at skin closure, and postoperatively at 24 and 48 hours). Total RNA was isolated from all samples, and gene expression analyses were performed with the Illumina HumanHT12 v4 microarray. Individual samples were randomly allocated into training (n = 149) and test (n = 141) sets to validate results. Results Randomization between the training and test set showed no significant differences in clinical parameters between the two sets. Supervised gene expression analyses indicated that inflammatory pathways were significantly increased in all treatments, and these pathways were increased in ONCAB vs MECC or OPCAB, with no difference between MECC and OPCAB, at 24 hours postoperatively by multiple testing approaches. By 48 hours postoperatively, differences in inflammatory gene expression were no longer detectable.

Avoidance of Dialysis in an End-Stage Renal Disease Patient Status-post Off-pump Coronary Artery Bypass Grafting
Certain benefits are clearly associated with the use of off-pump coronary artery bypass grafting (CABG) as compared with the on-pump CABG. The superiority is more evident in patients with multiple co morbidities including renal failure. We reviewed the medical records of a 67-year-old male with a past medical history that was significant for multiple cardiovascular diseases and new-onset end-sta...
متن کاملLess invasive coronary artery revascularization with a minimized extracorporeal circulation system: preliminary results of a comparative study with off-pump-procedures
BACKGROUND Coronary-artery-bypass-grafting (CABG) with conventional extracorporeal circulation (CECC) is associated with adverse effects such as systemic inflammatory response leading to a decrease in systemic vascular resistance and hemodynamic instability. Modern "less invasive" procedures have been established recently which potentially avoid negative side effects of CECC. The aim of this st...
متن کاملFrequency of Acute Kidney Injury in Patients Treated With Normal Saline after Off-Pump Coronary Artery Bypass Grafting
Introduction: Acute kidney injury (AKI) is a common postoperative complication of cardiac surgery, which is associated with an increased risk of morbidity and mortality. This study investigated the frequency of postoperative AKI in low risk adult patients undergoing off-pump coronary artery bypass grafting (CABG).Materials & Methods: All consecutive adult patients of American Society of Anesthe...
متن کاملOff-pump coronary bypass grafting is associated with less myocardial injury than coronary bypass surgery with cardiopulmonary bypass.
BACKGROUND Excessive myocardial necrosis following coronary artery bypass grafting is associated with adverse outcome. The present study was designed to assess the extent of myocardial injury after conventional coronary artery bypass grafting with cardio pulmonary bypass (ONCAB) compared with off-pump coronary artery bypass (OPCAB). METHODS Measurements of serum cardiac troponin T (TnT) were ...
متن کاملUse of Carbon dioxide versus air blower in on-pump beating-heart coronary artery bypass surgery
Introduction: The use of carbon dioxide blower has been recognized as the standard of care in patients undergoing beating coronary artery bypass grafting (CABG) due to higher solubility and lower risk of embolization. On the other hand, the compressed air blower has gone out of use since air can be easily trapped and is less soluble which can cause coronary embolism. The prese...
